Absence Note Form


A Note from the Attendance Office

In case of any absence from school, a student must bring a doctor's note or a written excuse note from a parent or guardian stating the reason for the absence. This excuse must be given to the homeroom teacher within 48 hours of the student's return to school. State law recognizes the following reasons as legal absences:

  1. Personal illness
  2. Death in the immediate family
  3. Recognized religious holiday
  4. Instances where attendance would be hazardous
  5. Service as a page in the Georgia General Assembly


The following are some reasons your child's absence may not have been excused:

  1. The excuse note was lost/misplaced. Please write notes on a full or half sheet of paper. Post-it notes and other small notes are much more likely to get lost which may result in the child's absence not being excused.
  2. The note cannot be read or the student cannot be identified. Please write legibly and put your child's full name on the note. If the note comes to the attendance office with only the name Ashley on it and no last name or teacher name, the attendance clerk has no way to identify which of a dozen Ashley's the note belongs to.
  3. The note did not list the reason for the absence. Also please list the date of the absence.
  4. The office has not had time to process the note. Notes go from the student to the HR teacher to the grade level office to the attendance office. Every effort is made to correct attendance in a timely manner.
  5. The note is still in your child's book bag. This is quite common.
  6. The absence may be for a reason that is not excused.
